If your wedding party members have Internet access, you can just direct them to copy their measurements to the online version of the form below after they have been measured. Please contact us if you have any questions.

Note: Use the following online form only if you have already taken the printed version to a professional tailor or formalwear shop and have been measured. This online form is provided for you to return your measurements via email.

Form Instructions
Step 1: Print the form
Print a copy for each male in your wedding party who will be wearing a tuxedo and lives too far from our shop to come in for measurements.

Step 2: Send the Form.
Mail the form to each gentleman with instructions on what to do.

Step 3: Get Measured.
Be sure to let them know to go to a professional tailor, men's clothing store, or some other formalwear shop to be measured.

Step 4: Return the Form. Copy your measurements to our online form (which comes to us via email), fax it, or sent the form via US Mail to Gentlemen's Choice.
